It might include the words 'Cheshire Regt.', but I'm not sure that suggests previous service with them bearing in mind that Question 10 asks: "Did you receive a notice, do you understand it. Who gave it to you?..."

Question 7 asks about previous service, and although he answers 'yes', he gives no further information.

It looks as though he received 'the notice' from someone in the Cheshire Regiment.

I agree - those details are of the person who gave the notice.
